Developing Data Systems for Care Coordination Using Patient-Centered Approaches

This All In: Data for Community Health project showcase webinar features two project teams developing a system that integrates data from sectors such as medical, behavioral, social services, and education to improve care coordination for special needs populations.

Altair is working with a health information exchange and discusses how to successfully engage the community in care coordination. Children’s Comprehensive Care Clinic is working to design a community health data ecosystem that:

  • Enables participatory care
  • Allows for integrated ‘story’ to enable workflow integration from multiple entities
  • Avoids documentation burdens
  • Utilizes “Human Centered Design” principles throughout
