Community resource referral platforms are touted as a solution to streamline social needs referrals by enabling health care providers to identify local community social service resources, refer patients to these resources electronically, and track the outcomes of those referrals.

This publication examines the implementation of a community resource referral platform, NowPow, in Trenton, N.J. between 2020 and 2022.
