Finding Shelter and Support Along the Road to Better Health

This article explores how Sacramento is using Medicaid funds to help its homeless population find housing under California’s Whole Person Care program. Pathways, a Medicaid-funded four-year pilot program, provides coordinated delivery of an array of physical and behavioral health services, as well as homeless outreach, social services, case management, and housing support, for some of the county’s most medically and behaviorally complex people experiencing or at risk of homelessness.

Pathways is Sacramento’s adaptation of Whole Person Care, a statewide pilot program offered through Medi-Cal, California’s version of Medicaid. This article is part of a series on transforming health systems published with support from the Robert Wood Johnson Foundation.
