This case study highlights how Light on the Hill uses a cross-sector data sharing system to address the health and social needs of its community. With support from Data Across Sectors for Health, Light on the Hill obtain a cloud-based data sharing mechanism to track: 1) outcomes of people receiving services and 2) type, impact, and number of services received.
